Hi,

As per general coding guidelines, ICD-10CM Z01.42 coded as ‘Primary Diagnosis Only’ and exempted from the diagnosis present on admission requirement. The selection of code is depend upon reason for the visit. If patient is coming only for cervical smear to confirm findings of recent normal smear following initial abnormal smear, then you can bill Z01.42. There are some payers who have specific guidelines for billing Z01.42, but in general there is no guidelines which says bill Z01.42 only for the third visit. Please check your payer policy.

Thanks
