We would like to know if this scenario would be "with abnormal findings". If a patient comes in for a 1 month well check and mother is concerned about baby being gassy. The baby is normal and healthy upon the exam and is breastfeeding. The doctor advises mother to discontinue dairy in her own diet to see if that would alleviate the gassiness in the baby. Would this be an abnormal finding?

No. To report this code, physician should examine the patient and during the exam, if the provider notices any abnormal finding in the patient, than only he/she can report this code.
