The title of the procedure is Trapezial arthroplasty, but this appears to be only a removal of the trapezoid (25210). OP excerpt: After getting through the skin, obtaining hemostasis, protecting the cutaneous nerves, the interval between the first and second compartment was identified. The dorsal branch of the radial artery was stripped off of the capsule. At this point, the capsule was excised and flaps were elevated exposing the trapezium. The trapezium was then cut with osteotomes in quarters and removed piecemeal. Xeroform and impacted into the cavity and then enclosed this over the capsule with 3-0 Vicryl.

CPT code 25210 (carpectomy, one bone) would be the appropriate CPT code. Trapezoid (one amongst the eight carpal bones) was removed. Arthroplasty was not performed as their are no details of the procedure mentioned in the submitted documentation. Generally arthroplasty of carpal bones involves reconstruction or replacement with prosthetic implants, which was not performed. Hence it would be appropriate to bill CPT code 25210 in this scenario.
