How would I code the following:
DIAGNOSIS:
1.Hardware in good position under fluoroscopic examination, no impinging screw at the distal radioulnar level.
2.ECU tendinosis, right.
3.Mild impinging dorsal central dorsal ulnar synovitic reaction, right.
4.Small scar body reaction at distal volar surgical wound, right.

PROCEDURE:
1.Excision of scar body reaction, distal surgical volar wound, 2 mm x 3 mm x 6 mm, right.
2.Decompression, ECU tendon (sixth compartment) and tendon debulking and retinacular reconstraint, right.
3.Diagnostic arthroscopy, right wrist, with debridement dorsal impinging synovial tissue, right.
