wound exploration and scar revision

Pediatric Posted Wed 28th of January, 2015 12:21:38 PM

how would i code wound exploratino and scar revision of the rlq wound.

Patient had surgery 2 years prior. wound is from a cecostomy tube removal. Patient had persistent drainage Now presents for wound exploration.

Attention was directed to the right lower quadrant wound which was noted deep with opposing skin folds. An ellipse of skin was incised encompassing this old wound and the skin folds. The underlying subcutaneous tissue was removed to the area of the fascia. This segment of skin and subcutaneous tissue was completely excised. No underlying lesions were noted. The subcutaneous tissue was then reapproximated with the use of interrupted 3-0 Vicryl sutures. The skin was then reapproximated with the use of buried interrupted intradermal sutures of 4-0 Monocryl.

I'm thinking 789.33 22900 or 11406 and 12032?
specimen description: skin abdominal wall 4.5x2.7cm
Thank you.

Debridement codes 11042-11045 would best match the above scenario. Plus use repair code 12032. There is CCI edit between these two codes. Append modifier 59 with debridement code.
