When billing 92004 and 67840, which modifier should be used? modifier 25 or 57?

CPT 67840 (Excision of lesion of eyelid (except chalazion) without closure or with simple direct closure) is a minor procedure with 10 day of global day, in which E/M service on day of procedure and during 10 day of post-operative service is not billable. Modifier 25 is appropriate when an E/M service is provided on the same day as a minor procedure (defined by Medicare as having a 0-day or 10-day global period).
