What is the Revenue code for 43999 for the Lap Band Adjustment

We are getting rejected by Medicare for the code 43999 for Lap Band Adjustment because we have no revenue code to attach to it. I looked extensivly for 2 days and can not locate it Please help us

Revenue codes only apply to providers who bill these services (like gastric band adjustment) to the fiscal intermediary or Part A MAC. Revenue codes do not apply to physicians, other professionals and suppliers who bill these services to the carrier or Part A MAC.

LCD for 43999 suggests that "Revenue code not applicable".

When the service is performed in hospital OR, you can try with rev. code 0360 (Operating Room Services - General).
