Need help in coding wellness exams for children. Child is scheduled for wellness exam and either comes in with, for instance, a cold or thrush and knows about it before the exam. How is wellness coded, with or without abnormal findings. Thank you.

The patient always comes with complaints during visits, but if provider found any abnormal findings during their examination, then only the provider can report it as abnormal finding.

Example: Patient complains for sore throat and during the examination, physician found that patient has throat infection. That's an abnormal finding, so they can report for abnormal finding code for it.
