How do you bill a planters wart, office visit,xylocaine 1%ndc,supplies?

If patient came in just to have his plantars wart removed, no office visit code is applied. Just code '17110' alone. The supplies are bundled into the '17110'. If physician did a separate E/M and wart removal then according to the documentation, you code 99212-25--->99214-25 along with the wart removal code.
