Wartenberg Syndrome

Hi,
I need codes for the following:
DIAGNOSIS:
1.Wartenberg syndrome, right (superficial radial nerve neuropathy at brachioradialis level).
2.Right DeQuervain’s.
3.Right intersection tendinitis.
4.Right wrist posttraumatic arthrosis with TFCC tear, chondral defect in the lunate, and partial substantial tear of the scapholunate ligament.

PROCEDURE:
1.Right superficial radial nerve neurolysis in the distal forearm and release of the overlying brachioradialis tendon.
2.Right first dorsal compartment release.
3.Right second dorsal compartment release.
4.Posterior interosseous nerve neurectomy in the distal forearm, right.
5.Arthroscopy, right wrist, with TFCC debridement, chondroplasty of the lunate, and debridement of the scapholunate ligament.
