You did not mention if you were ONLY giving the injection or was this done in conjunction with an office visit. Strictly for the injection I would use CPT 96372 along with DX codes of 2811 for B12 def and the main diagnosis code - i.e., celiac disease (5790) or Colitis) 5551. You cannot just use the J code. Hope this helps
