My Dr. removed leads and generator that has been in seven years and not working properly. He did not reinsert the generator, but placed test leads in again. Would I use 64585 and 64595 or could I use 64561 and the 64595? Thanks

Please elaborate your question. It is unclear to me which nerve he is using for lead insertion or removal.

As per above scenario, CPT codes 64585 and 64595 are correct.
