What would be the cpt code to use for a "clia waived" office that uses the multi drug screen cup?

New HCPCS code G0434QW(CLIA-Waived) or G0434

For Private Insurance Carriers(Non-Medicare/Medicaid)
New CPT code 80104QW(CLIA-Waived) or 80104

I also read up that the "unit" allowed per day is "one" for "80104" even if the kit being used is a 12 panel? is this correct?

I checked on "MUE" and didn't find "80104", but I did on "G0434" X1.
Please let me know.

Marilyn, I had read in one earlier FAQ regarding 2011 CMS reimbursement and that has not implemented yet I think, I had already answered in your previous query about 2 months before regarding this, but now when I went through in CMS site, there is no any updation or information of 80104, only found G0434.
