scalpel was then used to incise through the skin on top of the lesion. There was no identifiable capsule to the globular mass but several transverse veins were noted to be running across the center of the mass.  The dissection was carried down across both radial and ulnar neurovascular bundles. The underlying flexor tendon sheath was seen, intact.

Sorry not sure what that is nor do I have additional money for that. This is what I coded please tell me if you agree . scalpel was used to incise through the skin on top of the lesion. carried down across both radial and ulnar neurovascular bundles. The underlying flexor tendon sheath was seen, intact, and with no pathology. The globular area of fat that was beneath the skin changed lesion was removed using bipolar cautery. Cpt code 26111 I didn't see I &D What's your thoughts ?

As per documentation it becomes clear that excision of subcutaneous soft tissue of hand was performed. It would be appropriate to bill CPT code 26111/26115 as per the size of the lesion.

 

Use CPT code 26111 for excision of tumor of size equal to or greater than 1.5cm.

 

Use CPT code 26115 for excision of tumor of size less than 1.5cm.

 

Also, record do not clearly mention the depth of the excision. If the excision has been performed to the level of muscles (subfascial excision), use CPT code 26116/26113.

 

Use CPT code 26116 for excision of tumor of size less than 1.5cm.

 

Use CPT code 26113 for excision of tumor of size 1.5cm or grater.
