Good morning, our rad went in to perform a TIPS placement on a patient that became unsuccessful due to the main portal vein being unsuitable for placement. The rad performed the rt hepatic venogram, portal venogram with pressure measurements and performed a balloon venoplasty to try to expand the portal vein, but it didn't work so it wasn't placed. Are we able to code the 36481, 75885, 36011, 75891 and 37248 or do we code the 37182 with a 52 modifier? Thanks in advance for any help!

As per general coding guidelines, sometimes several unsuccessful attempts are made during the same operative episode to perform a procedure and finally the last attempt is successful. In this instance, only one unit of a single code would be reported for the procedure successfully accomplished, regardless of the numerous attempts. The unsuccessful attempts are considered a part of the successful procedure.
As per above mentioned report all attempts are made to perform TIPS procedure but failed, so the appropriate code is 37182-52.
