Doctor circles "90837" with 4 units and "90847" with 2units. Should I use a prolonged code and do I need to append a modifier somewhere between the codes?

Code 90837 does not come with "additional blocks of time." It is specifically time-based. 90837 is for 60 minutes only. 90847 also does not come with "additional blocks of time." If your Doctor is wanting to bill for more time with the patient due to crisis then s/he would bill code 90839 and use 90840 for each additional 30 minutes spent with patient. However, psychotherapy for crisis must show documentation that presenting problem is typically life threatening or complex and requires immediate attention to patient in high distress.
