Umbilical Granuloma Cauterized

What would be the appropriate documentation required in a procedure note for cauterization of an umbilical cord? My doctor just mentioned granuloma cauterzed with silver nitrate stick. Is this enough to bill procedure code (17250) for this procedure or should there be additional criteria in the note?

17250 is the appropriate code.

Physician Responsibility
The physician applies a small amount of silver nitrate stick over the lesion's tip and then destroys the lesion with a cauterizing device.
