Ultrasound Billing

What is the required documentation to charge an ultrasound? Chart has a photo of the ultrasound. No measurements taken, doctor wrote findings of the ultrasound in chart notes. Patient had missed AB. Is this a billable ultrasound?

If Ultrasound has real image as photo of the ultrasound and also provider has provided the finding in the chart you can bill this ultrasound.
