Tunneled hemodialysis catheter placement

Patient came for placement tunneled hemodialysis placement. But the procedure was discontinued due to occlusion of internal jugular vein.

Physiscian cannulated the right internal jugular under ultrasound guidance using the 18 auge needle and guidewire was placed into the right internal jugular vein. It only went down 4 cm. Contrast then injected for study.

Is 36410, venipunture will cover the procedure or should I code 36299 for unlisted procedure, vascular injection???

Please use code 36000 and 76937-26.

Code 36000 represents 'Introduction of needle or intracatheter, vein'.

Code 76397 represents 'Ultrasound guidance for vascular access requiring ultrasound evaluation of potential access sites, documentation of selected vessel patency, concurrent realtime ultrasound visualization of vascular needle entry, with permanent recording and reporting (List separately in addition to code for primary procedure)'
