Tumor resection

Pt. has history of neurofibroma 1 and diagnosed with sciatic nerve distribution tumors suspected to be schwannoma versus less likely neurofibroma. The surgeon wants to proceed with right posterior thigh approach for peripheral nerve tumor resection x 2. Please provide help with Cpt code.Thank you.

Hi, Excision of these tumors is typically reported with one of these codes. However, for more distally located tumors in which the surgeon plans to excise the schwannoma but not any bone, you might report 64788 (Excision of neurofibroma or neurolemmoma; cutaneous nerve), 64790 (... major peripheral nerve) or 64792 (... extensive [including malignant type]). So, 64790 is appropriate as sciatic nerve is major peripheral nerve. Thanks
