Tubal, D&C, and Cyst

I have coded the tubal as 58600, but he also preformed a hysteroscopy with D&C, AND aspiration of ovarian cyst. What modifiers should I use on what code and is there ANY bundle codes for any of these? Thanks

Thank you for the query.

In the above mentioned query, you have not mentioned the approach of the procedure, whether it was done through abdominal approach or vaginal approach.

Below I am mentioning both the scenarios.

When it done through abdominal approach:

1. CPT code 58600 (Ligation or transection of fallopian tube(s), abdominal or vaginal approach, unilateral or bilateral)
2. CPT 58558 with 51 modifier (Hysteroscopy, surgical; with sampling (biopsy) of endometrium and/or polypectomy, with or without D & C)
3. CPT 58805 (Drainage of ovarian cyst(s), unilateral or bilateral (separate procedure); abdominal approach) is getting bundle in CPT 58600.

When it done through Vaginal approach:

1. CPT code 58600 (Ligation or transection of fallopian tube(s), abdominal or vaginal approach, unilateral or bilateral)
2. CPT 58558 with 51 modifier (Hysteroscopy, surgical; with sampling (biopsy) of endometrium and/or polypectomy, with or without D & C)
3. CPT 58800 with 51 modifier (Drainage of ovarian cyst(s), unilateral or bilateral (separate procedure); vaginal approach)
