Transforaminal ESI

Would this be coded as 64483 only or 64483,64484? I'm including procedure note.
Left L4 and L5 nerve blocks via the transforaminal route.-
Under flouroscopic guidance a 25 gauge Chiba needle was advanced to a point just outside of the foramen. PA,lateral,and oblique flouroscopic views were used to confirm needle placement.There was no returnof blood with aspiration,I thus proceeded to inject Isovue 300 1.5 ml.There was clear spread of dye along the left L4 nerve root. I thus injected Bupivicaine 0.25% 1 ml and methylprednisone 20mg . This was repeated in the exact same fashion over the left L5 nerve.
Agan my question is would this be coded as a single level as 64483? Or with additional level as 64483,64484? Thanks for your help.

You should report CPT 64483 for your first-level injection. Report each additional injection separately with +64484 (... lumbar or sacral, each additional level [list separately in addition to code for primary procedure]).
