If an attempt was made to inject L4 and was unsuccessful, then L5 was injected successfully, would you code both levels? Would you code "64483" and "64484" with mod 74? (I code for the ASC)

Yes, it would be appropriate to report modifier 74 with the billed code.

Use modifier 74 with code 64483 and bill 64484 for additional level (L5) epidural injection.

We further researched into your query and want to give you additional information.

The above answer of billing codes as 64483-74, 64484 applies only when the documentation supports that provider had planned both level injection (l4-l5) and he was successful in only one.

However, if the provider had not planned both level injections. He first attempted L4 which fails and then does it at L5 level. In that scenario, you may bill only single level injection code, i.e. 64483.
