Transeptal/transsphenoidal approach to the pituitary gland

Elaine Posted Thu 24th of May, 2012 19:11:08 PM

Is there a code for just the approach?

After adequate general anesthesia was established the patient's nasal septum was injected with 1% lidocaine with 1:100,000 units of epinephrine. The nasal cavity was decongested bilaterally with 1:100,000 epinephrine solution on cotton pledgets. The patient was draped in a sterile fashion. The septum was deviated to the left with a large spur projecting into the left nasal cavity. A left hemitransfixion incision was made and a left anterior tunnel was elevated. A strip of cartilage was removed from the midportion of the septum over the maxillary crest. Bilateral inferior tunnels were created around the maxillary crest and the maxillary crest was removed with an osteotome. The nasal spine was also lowered with an osteotome. The majority of the bony septum was removed back to the sphenoid rostrum. The anterior face of the sphenoid was removed with adequate exposure to the posterior table of the sphenoid sinus and the area of the pituitary tumor. A self retaining pituitary retractor was placed. The resection of the tumor will be dictated by the neurosurgeon. At the end of the procedure Doyle splints will be placed. The hemitransfixion incision will be closed with interrupted 3-0 chromic sutures.
