TPT stenting

At that time the Laser was reintroduced and set at 60/25 and the SFA, POP, TPT, and proximal PT was treated with two passes. Total Tx time was 6:25 min and total pulses 10,237. Then a Supera stent was placed into the distal pop and into the distal SFA measuring 5 mm x 120 mm. Then a Xience stent was placed into the TPT and partially into the POP stent. The Xience stent was 3.5 x 38 and was post dilated to 20 ATM for max luminal diameter of 4 mm. I would code 37227 and 37231. Would this be correct for stenting of the pop and TPT?

Firstly superficial femoral artery stenting will not be coded separately and only popliteal, peroneal, and tibial artery stenting will be coded since popliteal, peroneal, and tibial arteries fall under beyond third order branches.

For popliteal artery stenting, you can select any one CPT code from following as per your documentation:

  • CPT code 37226 {Revascularization, endovascular, open or percutaneous, femoral, popliteal artery(s), unilateral; with transluminal stent placement(s), includes angioplasty within the same vessel, when performed} - - if stenting with angioplasty is being performed.
  • CPT code 37227 {Revascularization, endovascular, open or percutaneous, femoral, popliteal artery(s), unilateral; with transluminal stent placement(s) and atherectomy, includes angioplasty within the same vessel, when performed} - - if stenting with atherectomy with angioplasty is being performed.

  • CPT code 37226-52 (modifier 52 to indicate reduced service) - - if stenting without angioplasty is being performed.

For tibioperoneal artery stenting, you can select any one CPT code from following as per your documentation:

  • CPT code 37230 {Revascularization, endovascular, open or percutaneous, tibial, peroneal artery, unilateral, initial vessel; with transluminal stent placement(s), includes angioplasty within the same vessel, when performed} - - if stenting with angioplasty is being performed.
  • CPT code 37231 {Revascularization, endovascular, open or percutaneous, tibial, peroneal artery, unilateral, initial vessel; with transluminal stent placement(s) and atherectomy, includes angioplasty within the same vessel, when performed} - if stenting with atherectomy with angioplasty is being performed.
  • CPT code 37230-52 (modifier 52 to indicate reduced service) - - if stenting without angioplasty is being performed.
