total knee arthroplasty with autogenous bone graft

I am coding a total knee arthroplasty. (27447)

This patient has a large bone cyst that the physician removed from the medial femoral condyle.

The physician mixed Callos bone void filler mixed with local autogenous graft he took from the distal femoral cut. And than he filled the bone void with this mixture.

I don't think I am able to bill this, especially since I believe 20902 description states it needs to be from a separate incision.

Can you advise if I am correct in this.

Yes, you are correct. CPT code 20900-20902 can only be reported if the harvesting of autogenous bone was performed through a separate incision. Other than the primary procedure. You cannot bill it along with CPT 27447 in this case.
