TLIF w/ Partial Lateral Diskectomy

Nicole Posted Thu 20th of October, 2016 09:05:42 AM
The patient was placed in the LATERAL position for the first part of the operation. The doctor did an ANTERIOR lateral interbody fusion from L2-3 (22558). Then a partial diskectomy from the LATERAL position from L3-4 w/o Fusion (?). The patient was then placed in the PRONE position for the second part of the operation. Laminectomy, foraminotomy and facetectomy from L3-S1 (63047, 63048x3). Diskectomy was completed at L3-4 and then was continued down to L4-S1. L1-S1 POSTERIOR segmental instrumentation and fusion (22612, 22614). Transforaminal lumbar interbody fusion from L3-S1 (22633, 22634x2). 22558 (L2-3) Anterior Interbody Fusion _____ (L3-4) Anterior Partial Diskectomy without Interbody Fusion 63047, 63048x3 (L3-S1) Laminectomy (This is INCLUSIVE with the TLIF- Not Billing) 22612, 22614 (L1, L2) POSTERIOR Segmental Instrumentation and Fusion 22633, 22634x2 (L3-4, L4-5, L5-S1) Transforaminal Lumbar Interbody Fusion Also, Fluoroscopy (77003), Allograft (20931), Posterior Instrumentation (22842), PEEK Cages (22851x4) Do the above codes look correct? Is there a code that can be used for the Anterior Partial Diskectomy?

The diagnosis codes are: Scoliosis, Lumbar Stenosis, Lumbar Spondylolisthesis.... If this will help in regards to the codes. I have come up with these to bill: 22633, 22612-59, 22558-59, 22842, 22634 x2, 22851x4, 22614, 20931, 77003

For Anterior Partial Discectomy, the appropriate code will be 22224 (Osteotomy of spine, including discectomy, anterior approach, single vertebral segment; lumbar). Rest of the codes are correct. Thank you.

So, if it was just a partial discectomy without much bone work (other than removing the osteo hues laterally), would this still qualify for the 22224? Would it be appropriate to append a -52 modifier for reduced services, possibly?
Nicole Posted Wed 26th of October, 2016 08:15:21 AM
The doctor is questioning the code because he is not sure if it qualifies as an "Osteotomy". I appreciate the help!

Any suggestions on this? Bill with a -52 Modifier or without?
SuperCoder Answered Mon 31st of October, 2016 00:48:55 AM

Billing with modifier 52 will be appropriate for the partial procedure performed. Thank you.
