How would I code the following (other than the 26055):
DIAGNOSIS:
1.Right thumb arthralgia with instability at MCP level.
2.Recent onset, right thumb triggering.
3.Symptomatic distal dorsal radial metacarpal head exostosis.

PROCEDURE:
1.Right thumb A1 release.
2.Right thumb bony exostosis excision, distal dorsal radial metacarpal.
3.Reconstruction, radial collateral ligament with advancement for instability.

Reconstruction of collateral ligament can be coded as CPT 26541 if reconstructed with tendon or fascial graft, CPT 26542 if reconstructed with local tissue.
Metacarpal bony exostosis excision-26200 and A1 pulley release can be billed as CPT 26055 with modifier 59.
Diagnosis with ICD9-CM- 718.84, 726.91 & 727.03.
