Tethered Cord at T9-T10 with scar tissue at T12-L1

Mary ann Posted Tue 08th of October, 2013 17:04:52 PM

First, the T12 spinous process lamina was removed with decompression and scar tissue removed. T8 spinous of the lamina was removed with decompression and scar tissue was removed. At around T9, the cord was tethered to the dura with extensive scar. The dura was detethered. A dural patch was sutured in place. The only code I see is for release of tethered cord, lumbar. I saw a previous question regarding this issue with the advice of using 63200 and 63295...does this advice still accurate. I don't think 63295 is accurately reflected with this procedure. Would I be able to use 63271-laminectomy for excision of intraspinal lesions other than neoplasm, intradural, thoracic? He does talk about intradural exploration.. Thanks

For release of tethered cord, you report code 63200 (Laminectomy, with release of tethered spinal cord, lumbar). You may also report +69990 (Microsurgical techniques, requiring use of operating microscope [List separately in addition to code for primary procedure]) for the microdissection performed, if any.

Considering the removal of lamina , I would also inclined to use 63271
