Testosterone Cypionate 200 mg/ml NDC# 0009-0417-02

Debbie Posted Thu 05th of February, 2015 18:18:43 PM

when I look up this code it indicates I should code it 1 mg x 2000. But I only administer 1 ml which is 200 mg. What is the correct unit to bill?

Thank you for your question.

Please go ahead and bill J1071 for Testosterone Cypionate X 1 unit.

If the full dosage provided is less than the dosage for the HCPCS code descriptor specifying the minimum dosage for the drug, you should report one unit of the HCPCS code for the minimum dosage amount.

Hope it helps answer your query.
