Tegaderm Patch with Baclofen

A patient comes in to office with low back pain and my physician wants to apply a compounded drug (Baclofen, Lidocaine and Marcaine (cream) to area and apply tegaderm for 20-30 minutes. Does anyone know how I would bill this??

Do you know name of the cream used which is a compound of Baclofen, Lidocaine, and Marcaine ? Or all three individually applied on the affected area ?

We use a pharmacy which compounds those 3 drugs together. We receive the cream in one single syringe.
