Talonavicular Nonunion

How would I code the following:
DIAGNOSIS:
1.History of left ankle replacement and subsequent revision with plantar medial bony prominence over the medial talus.
2.Nonunion of talonavicular joint with flatfoot deformity.

PROCEDURE:
1.Excision of bony osteophyte and hardware removal from the medial talonavicular joint, left.
2.Takedown of talonavicular cuneiform joint nonunion with bone grafting and refusion.

Well, it is a revision of the fusion, so CPT 28899 seems appropriate where hardware removal will not be billed separately. Use ICD 733.82 for non-union. Hope it helps!
