The provider wrote 2.2 + 1.0 The diagnosis 701.4. Are CPT 12032-59 and 11423 the correct CPT to bill this procedure?

Rather than 2.2 + 1.0 indicating addition of two, it should have been 2.2 x 1.0. Taking 2.2 as the maximum size of lesion, use of code 11423 is correct for the excision of keloid from scalp, neck, hands, feet, genitalia.

Code 11423 represents 'Excision, benign lesion including margins, except skin tag (unless listed elsewhere), scalp, neck, hands, feet, genitalia; excised diameter 2.1 to 3.0 cm'.

Code 12032 represents 'Repair, intermediate, wounds of scalp, axillae, trunk and/or extremities (excluding hands and feet); 2.6 cm to 7.5 cm'. You may use this code when the provider documents the size of repair of defect in size 2.6 cm to 7.5 cm.
