Dear Supercoder team My doctor has performed following procedures, please clarify me about coding. Left Subclavian Angiogram by selective placement in Subclavian Left subclavian lesion on left subclavian angiogram of 80% to 85% lesion in the proximal segment close to the ostium with 60 mm gradient across the lesion. Successful intervention of the significant lesion with balloon expandable stent. My coding is following. 37236 36215-59 75710-26,59 or 37236 36225-59 This is a Medicare patient, so I want to have the accurate answer. Oscar M

As per the documentation provided, appropriate CPT codes would be 37236 and 36225. Do not append modifier-59 to procedure code 36225 as there is no CCI Edit between both the codes.
