Hi, Subacute care is a distinct form of health care service that focuses on providing the skilled medical care needed to transition individuals from the acute care setting. Subacute care may be rendered in a freestanding facility or in a designated unit of a general or rehabilitation hospital. In case of inpatient rehabilitation, The procedure codes are (99221-99233) and the POS code for IP Rehab is 61. 99238-99239 These codes are to be utilized by the physician to report all services provided to a patient on the date of discharge, if other than the initial date of inpatient status. To report services to a patient who is admitted as an inpatient and discharged on the same date, see codes 99234-99236 for observation or inpatient hospital care including the admission and discharge of the patient on the same date. To report concurrent care services provided by a physician[s] other than the attending physician, use subsequent hospital care codes [99231-99233] on the day of discharge. In case of an outpatient rehabilitation, use codes 99201-99205 for new patient and 99211-99215 for established patient and POS code for OP rehab is 62. Hope this helps! Thanks.
