If Physical exams are not required to bill for store-and-forward consults, are the other elements such as HPI, PFSH, ROS and Medical Decision making required? Do we take whatever provided elements the MD has provided to determine a consult code as we do with a normal inpatient consult (99251-55) and simply apply a GQ to it?

Modifier GQ denotes healthcare services provided via an asynchronous communication method. Asynchronous communication does not take place in real time. An example of delivery of a service via asychronous communication includes a provider at a distant location, even in another state, who receives video clips, still images, x-rays, MRIs, EKGs and EEGs, laboratory results, audio clips, and text transmitted across a secure network and then transmits the report of his reading of the images for later review by the patient's primary care provider.
The store and forward teledermatology required the clinical information, such as history of present illness and transmission of the information from referring provider to dermatologist for asynchronous review.
In the Consult Process of S&F Teledermatology the Primary Care provider sees the patients’ condition, takes the digital photograph then the patient’s history and photographs transmitted to dermatologist and lastly the dermatologist reviews the case and provide the recommendation to the PCP. Hence, you need to consider all the elements provided by the MD with modifier GQ.
