How would I code this scenario:

Pt stepped on nail 1 week ago. Examination showed healed puncture wound. Did have minimal pain. Should I use the pain only or is there something I could use for the accident?

You should use 883/893 category for open wound nail, but as the wound is completely healed, you should not code it. There is no specific code for healed puncture wound, so you can bill for pain here.
