SSC excision w/split thickness graft reconstruction

How would the following be coded: 3.5cm SCC lesion of left palm & 1.5x2 cm SCC lesion of left ring finger were excised down to subQ tissue in the hyperthenar fascia of the palm & the dorsal ring finger. A 21/1000 split thickness skin graft was then harvested using a zimmer dermatome. The graft was then tailored to fit both defects & inset using a combination of simple & running 4-0 chromic sutures. Owens gauze was applied to the recipient sites followed by a bolster dressing. The donor site was dressed w/xeroform followed by 4-0 chromic sutures x4.
Would codes 11624, 11622 & 15120 w/dx 173.62 be appropriate/correct?
