My surgeon originally did a TLIF on the patient one month prior. The patient then had a CT scan showing migration of the bone graft as well as displacement of the interbody spacer at L5-S1. The doctor took him back to the OR and removed the rod and screws at L5-S1 and L4-5, at l5-S1 he removed the excess bone graft material and pushed the interbody spacer back into place. Then at L4-5 he again removed excess bone graft. Screws and rods were put back in place and poterolateral bone graft placed and patient closed. I am thinking 22849 but would that include removal of the excess bone graft?

The appropriate code for this scenario is 22849 and CPT code for removal of bone graft 20680 is not included in this code.

I was also thinking that perhaps codes 22849 and 22830 would be suitable. What is your opinion?

Both the CPT codes 22849 and 22830 seems to be appropriate for the scenario and there is no bundling found.
