Please verify that I have this coded correctly and help with questioned codes.
Revision of lumbar spine fusion with peek implant, set screws and rods were removed at L5-S1 (22852) does this mean revision? or is there another code for revision?
External neurolysis L5-S1 (64714).
Repositioning L5-S1 Peek (????) part of revision?
Laminotomines bilateral with bilateral discectomy @ L3-L4 (63030-50).
Bilateral posterior lumbar interbody fusion with peek implants at L3-4 (22633) and (22851)
Posterior fusion with bone graft L3-4, L4-5 and L5-S1 only bill this twice because L3-4 would be part of 22633 so just 22634 x2)?? or would I use 22612 and 22614 since only a posterior was done at these 2 levels L4-5 and L5-S1
Stabilization L3-4 with pedicle screws (would I use 22842)??
Impervious membrane over dura at L3-4 (15777)
Bone graft (20930).

22633x1, 22614x2, 22842x1, 22851x1, 15777x1, 20930x1, 64714-59 x1. 63030-59 x1 (if your documents supports that laminotomy was done for decompression other than fusion preparation). CPT 22852 cannot be billed, as removal of rod and screws performed at L5-S1 level, where revision of fusion has been done.
