Specific documentation requirements for 92014

Greetings, I'm inquiring about specific documentation for 92012 vs 92014. For instance, how many diagnosis are needed to document 92014. Do all 92014's need a diagnosis with initiation of treatment or can it have a continuation of treatment as well?

Hi Angela,

As per above documentation it can have continuation of treatment as well if prescribed by physician.

Further, If we have to code 92014, we will have to follow comprehensive speciality sheet for eye. Below is the CMS link that will guide you in comprehensive E/M. (follow page 23 and 24). It states Perform all elements identified by a bullet; document every element in each box with a shaded border and at least one element in each box with an unshaded border.
