How would I code the following: DIAGNOSIS: 1.shoulder adhesive capsulitis/frozen shoulder. 2.shoulder severe synovitis. 3.shoulder subacromial impingement syndrome with partial rotator cuff tear of the supra and infraspinatus. PROCEDURE: 1.shoulder arthroscopic subacromial decompression including debridement of partial rotator cuff tear, bursal sided. 2.shoulder arthroscopic subtotal synovectomy including 360 degree capsular release and synovectomy of the glenohumeral joint.
