When the doctor said's he has shaved or excised a lesion 6 or 8mm, should you convert that to cm?

Absolutely; these code descriptors are mentioned in centimeters.Convert the mm size to cm size for proper code selection for eg.6mm is 0.6 cm and 8mm is 0.8cm.
