As per the documentation the appropriate code seems to be 46020. But for appropriate answer please provide some detail of procedure and diagnosis.

As per coding guidelines seton tightening is considered to be included in the global period of the major procedure. As per your given scenario, the patient is coming for seton tightening after the global period of the major procedure, hence you can bill the service with an unlisted code i.e. 46999 (Unlisted procedure, anus); as there is no specific code available for seton tightening only.
