Selective/Non Selective Wound Debridement

Karin Posted Tue 31st of January, 2012 15:09:47 PM

We have therapists in our hospital that provide selective and non selective wound care (97597/97602). Our therapist use Silvermed Gel A6248 when doing the wound cares on some of the wounds. We are wondering if this is appropriate to bill since the codes 97597/97602 states "including topical application". There is no reference as to what that may include. I have check several LCDs and still not sure on if that is a billable item. We also do not have a supplier number for DME. Thank You!

Topical application is included in both, so is not a factor to differentiate if it is a selective wound care or non-selective wound care procedure.
The basis of selective and non-selective wound debridement procedure depends on the method of care taken.
If the debridement procedure, including topical application, also include application of high pressre waterjet, use of a whirlpool and code is based on area of debridement, it is selective debridement.
If the debridement, including topical application doesn't involve the significant features of selective debridement(high pressure debridement technique), but involves wet-to-moist dressings, enzymatic abrasion, etc including topical application and the code is not based on area but by method as described, then it is non-selective debridement.

Infected areas particularly cellulitis, pressure ulcers, and ulcers are compatible for these CPT codes.
