Op note states selective nerve root injections at L4-L5, L5-S1. Under direct visualization needle inserted into foramen at the two levels, injection Marcaiane and Dexamethasone. Patient tolerated all four injection well. Could you please tell me the correct CPT codes and correct units for reporting. Thanks so much

If selective nerve root block is performed on both right and left side, report as follows:

L4-L5- 66483-RT, 64483-LT
L5-S1- 64484-RT, 64484-LT
