2. CPT code for angiography of both vertebrals and basilar artery would be 36226-50 (modifier 50 to indicate bilateral vertebrals)

3. CPT code for left thyrocervical trunk would be 36216-59 (modifier 59 to indicate distinct procedure as per CCI guidelines).

4. CPT code for right thyrocervical trunk would be 36217-59 (modifier 59 to indicate distinct procedure as per CCI guidelines).

5. CPT code for angiography of right ilio-femoral artery would be 75710-RT (modifier RT to indicate right side).
