Scar revision post carpal tunnel surgery

Can you please help me with the following surgery: Scar revision with excision of painful suture granuloma distal aspect of the carpal tunnel release scar. Patient is 3 months status post (out of global) and is now returning for scar revision. Incision is made through skin and subQ only. No foreign body was found. Thank you for your help!

Scars revision usually billed with complex repair codes. Since the patient is out of global period, scar release can be billed with CPT 13131 (Repair, complex, hand; 1.1 cm to 2.5 cm). In this procedure, the provider makes a cut deeper through the entire epidermis, repair is more difficult. This procedure involves more than layered closure, involving such elements as extensive undermining, stents, or retained sutures. Preparation for this procedure includes creation of a defect for repairs (e.g., excision of a scar requiring a complex repair) or the debridement of complicated lacerations or avulsions. Check your op-report and select the CPT according to the size of the scar revision performed; CPT 13131 is for 1.1 cm to 2.5 cm, CPT 13132 is for 2.6 cm to 7.5 cm and CPT 13133 is for each additional 5 cm or less.
