What CPT code/codes would be appropriate for the this procedure (the ICD-9 given is 998.32):

Location and size: Nose 0.8 x 1.6 cm
Anaesthesia: !% xylocaine with epinephrine 1:100,000 buffered, 4.00 cc
Pre- Op: Informed consent reviewed with patient. Treatment alternatives d/w pt. Consequences of refusal of treatment d/w pt. Risks and benefits d/w pt incl. but not limited to scarring, infection, bleeding, recurrence, need for further treatment, Patient's questions answered to his/her satisfaction. Signed consent in chart.
Procedure: The area was prepped and draped in sterile fashion. Incision was made to the level of the subcutaneous fat with a #15 blade scalpel. The surgical flap was made using Adson forceps and Stevens tenotomy scissors. Hemostasis was accomplished with electrodesiccation and pressure. To achieve tension-free approximation of wound edges, undermining laterally was accomplished with blunt tenotomy scissors, being careful to avoid nerves and vessels
Closure: 4-0 Vicryl was placed in the deep dermal-fat junction, 4-0 Ethilon was placed in a running manner in the epidermis., Steri-strips were placed across the incision., Polysporin and a pressure dressing were placed.
Final Length of incision: 2.00 x 4.00 cm
