How would I code the following:
DIAGNOSIS: Rheumatoid arthritis with forefoot pain and subluxation of the MTP joints with hammertoes, second and third.

PROCEDURE:

2.Second, third, fourth, and fifth metatarsal head resections, right.
3.Closed osteoclasis of the second and third PIP joints, right.
4.Pinning of second, third, fourth, and fifth toes across the MTP joints.
